This form is for grantees that received a project grant from Raft Foundation or one of its fiscally sponsored projects. Details on the frequency and scope of each report are included in your grant agreement, in section 2 of the terms and conditions.
The person filling this out must be an authorized representative of the grantee. You will be asked to sign at the end.
The legal name of the person or organization that signed the grant agreement with Raft Foundation.
Also known as the grant "Purpose." Leave blank for general support grants.
The grant narrative should be 500 words or more and describe what was accomplished during the reporting period by Grantee (or, if appropriate, each subgrantee, if any) in furtherance of the Purpose.
Please provide a budget detailing Grantee’s expenditure of Grant funds during the reporting period. Or, if this is the final report, include all funds spent, since the beginning of the grant.

Please share any updates, roadblocks, issues of concern, and any other information that is materially significant to the grant purpose.
